Online Form / Survey Creation
Ever want to get more information from your audience? Ever want to create surveys? The web creation group will work with you to create a online interactive forms. There are no cost for the form creation unless we generate any physical materials such as paper. Depending on the type of form we create for you there might be some paper that is used during the testing phase. The price for the paper is nominal (usually a $1.00 per page) but check with us for current pricing on paper if there is a concern. There are three different types of forms we can create.
- PDF Pritable Forms
- These forms are mostly put online for users to download and print on their printer. They are used mainly where a signature is required. In most cases these PDF documents can have "fill in able" areas where a user can type in their information and then print out the form to sign later. We can also create fields in the PDF's where calculations are required..
- Mailto Forms
- These forms are online and used to get answers to questions from users. The results will be sent to you through e-mail. One e-mail per submission will be generated. This is the easier of the forms to create but can cause a lot of e-mail to appear in your in box. We highly recommend that you set up your e-mail package to filter the resulting email into a special in box of your own.
- Writeto Forms
- These forms are online and used to get answers to questions from users. The results will be sent to and stored on our web server in a tab delimited text document. No email is sent to you. At a time of your choosing you can download the text document using a FTP program such as WSFTP. Provided that you don't need the information on a daily basis we can download the information for you and send it to you through email. The benefit to this form is that the results can be opened in Excel and the results be manipulated how ever you want.
